Doctors Discuss Typical Side Effects of Weight-Loss Medications

Rising Popularity of GLP-1 Medications and Their Side Effects

In recent times, medications like Ozempic, Wegovy, and Zepbound have gained significant popularity. Initially used for managing type 2 diabetes, GLP-1 medications are now recognized by the FDA for weight loss. Interestingly, they also appear to lower the risk of certain cancers, heart disease, and cognitive decline, among other health issues.

“They function by reducing hunger, slowing stomach emptying, and promoting a hormonal reduction in fat cells,” explained Dr. Holly Lofton, a specialist in obesity medicine at NYU Langone Health.

While these drugs have helped countless individuals shed pounds and mitigate severe health risks, it’s essential to acknowledge that, like anything, they come with potential risks that aren’t often highlighted alongside their benefits.

Dr. Naomi Parrella, head of lifestyle medicine at Rush University in Illinois, described these medications as “very, very powerful.” She noted that their strength is tied to the potential side effects.

Not everyone on GLP-1s will experience side effects, but when they do occur, they can vary from mild discomfort to severe health threats.

Common Gastrointestinal Side Effects

“The most frequently reported issues are gastrointestinal: nausea, constipation, loose stools, and vomiting,” Lofton stated. However, these symptoms don’t affect everyone. In clinical trials, around 44% of participants reported nausea.

“These drugs mimic a natural hormone and amplify it, so individual responses can differ significantly based on one’s sensitivity to such hormones. As a result, the most common side effects stem from how these medications affect food movement through the digestive system,” Parrella explained. This slowdown often leads to symptoms like acid reflux and heartburn.

Risks of Dehydration and Nutritional Deficiency

Many users of GLP-1s might not feel hunger or thirst as frequently, which can create challenges. “Some individuals may not consume enough food or fluids, risking dehydration and a lack of essential nutrients,” Parrella added. The medication often makes users feel full sooner, leading to reduced consumption of vital nutrients.

More Serious Health Risks

One severe potential side effect is pancreatitis, an inflammation of the pancreas. “This medication aims to stimulate the pancreas to release more insulin when foods raise blood glucose levels,” Parrella noted. The increased workload on the pancreas might unexpectedly cause pancreatitis in some patients.

There are also links between GLP-1 use and gallstones or gallbladder inflammation, Lofton mentioned. Excessive vomiting can lead to dehydration, potentially resulting in kidney failure. Furthermore, intestinal obstruction, where food gets stuck and stops moving through the gut, can occur, particularly due to dehydration.

These serious health concerns often share overlapping symptoms with milder side effects, which can make diagnosing issues more complicated. Parrella also highlighted a rare but critical risk of gastroparesis—the stomach’s inability to move food. This is a permanent condition that doesn’t resolve after stopping the medication.

Dental Health Concerns

Dr. Elizabeth Walton, clinical director at Risas Kids Dental, pointed out that GLP-1s can lead to increased gum disease and dental decay. The medications reduce saliva production, which is vital for oral health. “A lack of saliva can expose teeth and gums to harmful bacteria, increasing the risk of dental issues,” she said.

Guidance on Reporting Side Effects

If patients experience any vomiting, even once, Lofton advises them to contact their doctor. “Vomiting can signal more severe adverse effects,” she added. It’s essential to discuss any persistent or worsening symptoms with a healthcare professional.

While many individuals share their side effects, this doesn’t normalize them. Parrella noted that some patients may hesitate to report discomfort for fear of losing their prescriptions, but ignoring these signs can lead to more severe problems later.

Tips for Managing Side Effects

When taking GLP-1s, it’s crucial to stay hydrated and focus on protein intake to maintain muscle mass, Parrella said. Incorporating strength training can further aid muscle maintenance. Additionally, stopping food intake a few hours before bedtime can help prevent issues like heartburn.

Long-Term Risks Remain Uncertain

“We lack extensive data on the long-term effects of these medications, which means many risks are still unknown,” Parrella cautioned. Questions abound regarding potential cancer links, effects of discontinuation, and interactions with individuals outside clinical trial demographics.

Patients should closely monitor their bodies’ reactions and maintain open communication with their doctors. Lofton emphasized the importance of considering the risk versus benefit ratio, particularly for those without existing weight issues.
