My Daily Observation: The True Price of the E-Bike Surge

Understanding the Risks of E-Bike Rides

As someone who performs oral and maxillofacial surgery focused on facial trauma, I have witnessed how a seemingly pleasant e-bike ride can swiftly escalate into a serious medical crisis. E-bikes are a great way to get around — they’re efficient and convenient. However, their speed and weight can turn accidents into major issues, especially when it comes to injuries to the face and head.

Every day, I reconstruct faces damaged by trauma, and I’ve noticed an increasing number of these cases are related to e-bikes.

The scenarios are becoming all too recognizable. A teenager biking home from school, a parent enjoying a weekend ride through the neighborhood, or a commuter speeding along a coastal route where no one expects such speed from a bicycle. These bikes are often high-quality, well-engineered machines, but the protective gear—if it’s even worn—seems to be an afterthought.

In my operating room, I often see severe injuries: fractured jaws, shattered orbital floors, displaced maxillae, and teeth either knocked out or embedded in bone. It’s important to remember that faces don’t just bounce off asphalt; they take the full impact.

The Numbers Are Concerning

Since Florida introduced its updated system for classifying electric bicycles in July 2020, allowing them wherever regular bicycles are allowed, the rise in usage has been remarkable. This trend is particularly notable in affluent neighborhoods where owning an e-bike has become common, often serving as a teenager’s first taste of independence.

Unfortunately, crash records in Florida have historically struggled to consistently identify e-bike incidents. While it’s tough to pinpoint the exact number of accidents since the law was enacted, a recent analysis revealed 1,725 e-bike crashes resulting in various levels of injury—70 of which were fatal. The total of documented e-bike injury crashes surged from 158 in 2021 to 825 in 2024, highlighting a rapidly escalating issue.

This troubling trend shows no signs of slowing down.

Why Facial Injuries Matter

Unlike a broken wrist, which may heal and eventually be forgotten, facial trauma has a lasting impact on those affected.

The process of reconstruction often involves multiple surgeries over several months. Patients may need their jaws wired or plated, alongside dental procedures like implants and grafts. Many families are taken aback when they discover how much of this reconstructive work isn’t covered by typical medical insurance, pushing them to take on significant costs. These injuries often lead to weeks on liquid diets, pulling kids out of school and adults away from work, and the challenges of regaining normal speech and eating functions. Plus, there’s the constant reminder in the mirror of the injury suffered.

I mention these financial and cosmetic realities not to be insensitive, but because they often lead families to reconsider their decisions—like the choice to forgo a helmet for the sake of convenience.

Preventative Measures

Always wear a good helmet. It should fit snugly, meet safety standards, and be replaced after any significant hit. Florida’s Class 3 e-bikes can reach speeds up to 28 miles per hour—equivalent to motorcycle speeds, which ordinary bike protection may not adequately handle. At these velocities, riders should think about helmets that offer more coverage for the face and jaw. If you’ve invested a lot into the bike, spending a couple of hundred on a proper helmet is a wise choice.

Watch your speed. Be cautious at intersections, driveways, and areas where visibility is limited. Speeding compromises your reaction time and increases the potential force of any impact.

Stay visible and focused. Use front and rear lights, wear bright clothing, obey traffic laws, and never ride with your phone in hand.

Understand your bike. Get familiar with your e-bike’s acceleration and braking in a safe area before hitting busier streets. The way these bikes accelerate can catch even experienced cyclists by surprise.

Keep your bike in good condition. Check tires, brakes, and lights regularly. A mechanical issue that might be just a nuisance at slower speeds can lead to serious consequences at higher speeds.

When to Seek Immediate Care

After any face-related crash, it’s critical to look for significant swelling, bleeding, loose or misaligned teeth, numbness, or trouble opening your mouth. Sometimes, a jaw fracture may be indicated merely by an odd feeling in your bite; this is often overlooked for days.

E-bikes offer fantastic transportation options, and I’m not suggesting anyone should give them up. I do expect the same caution you would apply to any motorized vehicle: wear proper protection, manage your speed, remain visible and alert, and remember how quickly things can change.
