According to local authorities, the artist, whose real name is Isaac Freeman III, was headlining the Green and Gold Party in Hamden, Connecticut when he suffered a health emergency.
Social media footage of Freeman entertaining the crowd at Town Center Park shows him taking to the stage in the DJ booth and disappearing from the crowd's view.
According to multiple reports, people could be seen performing chest compressions behind a screen during the DJ's set while other performers onstage led the crowd in prayer.
TMZ Authorities were called to the park around 8:33 pm and reported that his concert was advertised to begin at 7:15 pm.
Freeman was taken off the stage on a stretcher and taken to a local hospital about six miles north of the park.
Hamden Mayor Lauren Garrett posted on social media that Fatman Scoop was experiencing a medical emergency and was being taken to the hospital.
“Please keep him in your thoughts and prayers,” Garrett posted on Facebook.
The rapper's tour manager, DJ and producer, Birch Michael, confirmed Freeman's passing on social media early Saturday morning.
“It is with great sadness that I announce the passing of Isaac Freeman III, known as Fatman Scoop,” Michael wrote. “I honestly have no words… You took me all over the world and let me perform on some of the biggest and most amazing stages on the planet. I truly appreciate all the things you taught me. [sic] He is the one who made me who I am today.”
The New York musician rose to popularity after his song “Be Faithful” garnered millions of plays.
He was also featured on Missy Elliott's “Lose Control,” which peaked at number three on the Billboard Hot 100 in October 2005.
Freeman also appeared on Mariah Carey's “It's Like That” and has written songs with Timberland & Magoo, Nick Cannon and Skrillex.
