One person was killed and several others were injured when a tour bus driver mistakenly pressed the brake pedal for the gas pedal at a cruise pier parking lot in Honolulu, Hawaii.

The fatal crash occurred at 10:24 a.m. local time Friday, according to Honolulu Emergency Medical Services and the Honolulu Police Department.

Authorities said the 57-year-old male driver was dropping off passengers at a cruise port pier when a bystander alerted him that the vehicle was moving forward.

The man immediately reacted, jumped onto the shuttle bus and attempted to stop the vehicle in progress.

According to police, the man hit the accelerator instead of the brake, causing the car to crash into two concrete fences and 11 pedestrians.

Three pedestrians were transported from the scene to a local medical center for treatment. One person was in critical condition and the other two were in good condition.

One woman died from injuries at the scene.

Two other pedestrians were taken from the scene to another local hospital in good condition, police said. The other six pedestrians refused medical treatment at the scene.

Authorities are continuing to investigate the crash, but said speed is not immediately considered a “factor” in the crash.

Police said it was unclear whether drugs and alcohol contributed to the accident.
