For instance, if family members are busy preparing dinner, a person might wonder, “What are these people doing?” They struggle to grasp the purpose behind the meal preparation, as noted by an expert. This confusion can lead them to feel like they are in the wrong place.

As a result, restlessness and agitation may follow. “They often pace back and forth or rock in their chair—anything that indicates a lack of calm,” another expert explained.

An individual might also experience anxiety, irritability, or even aggression, along with challenges in falling asleep or staying asleep. Family members might notice that the person wanders at night, flipping their schedule, so to speak, by being active when they should be resting.

It’s worth mentioning that these symptoms can vary from one patient to another since there are multiple types of dementia. For example, those with Lewy body dementia might have more visual or auditory hallucinations.

Distinguishing sundowning from delirium is crucial, as the symptoms may appear similar. Delirium can indicate a serious, sometimes life-threatening condition. Understanding its root cause is vital. “Disorientation due to delirium could signify a severe issue, like a significant infection, high blood pressure, or even a stroke in older adults,” one expert pointed out.

The key difference is that sundowning typically occurs around sunset or during a specific time of day, while delirium can emerge suddenly at any time. “Once the cause of delirium is identified and treated, it usually diminishes,” the expert clarified. On the other hand, sundowning tends to repeat, though there are strategies available to help manage its symptoms.
