On Thursday, former House Speaker Nancy Pelosi expressed her commitment to advocating for gender-affirming care for transgender youth at a national level. This statement came during her visit to the San Francisco VA Medical Center, where she discussed various improvements to the healthcare system aimed at protecting veteran services from potential cuts by House Republicans.

When questioned by reporters about California’s recent suspension of gender-affirming care, Pelosi reflected on the implications. “It’s something I work at a national level and I hope we can provide gender-affirming care for trans kids. That’s sad for us… I don’t know how we can impact what we do in the White House and Congress all over the country,” she commented.

She noted the presence of a transgender pride flag outside her office—a visual reminder of support from some colleagues. Still, she recognized that many Republicans in Congress oppose medical transitions for transgender youth, which adds to the challenges advocates face.

Earlier this month, over a dozen officials from Democratic-led states launched a lawsuit aimed at securing access to gender-affirming procedures for those under 19. The case, filed in a Massachusetts federal court, argues that the Trump administration aims to impose a nationwide ban on these essential services by threatening legal action against providers.

In the lawsuit, President Trump, Attorney General Pam Bondi, and the Justice Department are named as defendants. The complaint contests Trump’s January executive order and other government memos that limit support for gender transition treatments.

Pelosi’s office indicated that discussions surrounding this lawsuit have been part of the broader conversation about supporting transgender youth.

The Justice Department under Trump has reportedly been focusing on providers of gender-change procedures for minors. Recently, some healthcare institutions, including Kaiser Permanente and Children’s Hospital Los Angeles, have started suspending or ceasing services related to gender transition for patients under 19, reflecting the chilling effect of these legal developments.

Multiple states already have laws in place that restrict or ban sex-change surgeries for minors. Lawsuits have been filed in various states, including California, New York, and Massachusetts, aiming to challenge these restrictive measures.
