At 48, Johnson is convinced that enhancing his sexual performance—aiming for something on par with teenage vitality—requires bold strategies. He takes Cialis, participates in penile Botox treatments, and even undergoes low-intensity shockwave therapy to boost blood flow and improve overall function.
“Nighttime erections are crucial health indicators,” Johnson stated in 2024. “If men aren’t experiencing erections at night, they face a 70% higher risk of premature death. It’s a predictive marker.”
While Johnson is certainly not alone in his quest for longevity, his stringent regime is noteworthy.
Other fitness enthusiasts, like 44-year-old Ben Greenfield, have also caught attention for their approaches, dabbling in sound wave therapy and exploring stem cell injections to enhance sexual performance.
This raises interesting questions: Is penile health really a reliable gauge for longevity? Are these biohackers strategically planning for the long haul—or overexerting themselves in pursuit of an ideal that may be out of reach?
The Post consulted two experts, who shared their insights into the emerging trend of “penile biohacking” and offered advice for maintaining sexual vitality over the years.
How Penile Health Reflects Overall Well-Being
“What constitutes penis health? It’s often evaluated through various metrics such as Erectile Dysfunction (ED), Orgasmic Function, Sexual Desire, and overall satisfaction,” explained Dr. Ryan Welter, an expert in regenerative cell therapy. He noted that ED is increasingly recognized as an early indicator of cardiovascular issues, potentially appearing years before more serious heart conditions.
The arteries in the penis are notably smaller than those that supply blood to the heart and brain, making them early indicators of plaque build-up.
Factors like high cholesterol can hinder blood flow, complicating the physiological process necessary for erections. Moreover, nerve damage, linked to conditions such as diabetes and multiple sclerosis, can disrupt signals between the brain and the pelvis.
Additionally, hormonal imbalances, particularly low testosterone, play their part as well, leading to decreased sexual desire and issues with achieving or sustaining erections.
“Happiness, too, is a strong predictor of longevity,” Welter emphasized. “If someone is experiencing sexual dysfunction or dissatisfaction, it may indicate deeper issues that need addressing.”
Recognizing the Signs of Erectile Dysfunction
“Many men notice that their erections aren’t as firm as they used to be, and it becomes more challenging to sustain them,” noted Chris Bustamante, a cosmetic nurse and founder of a Men’s Clinic in New York City. “There can also be notable changes in nocturnal and morning erections.”
Factors such as delayed ejaculation or the opposite can vary with age. Sudden fluctuations in erection quality or frequency—especially when coupled with symptoms like rapid weight gain or fatigue—warrant an urgent medical consultation, Bustamante advised.
Tracking both nighttime and morning erections—along with their firmness—can provide useful insights.
Johnson utilizes an Adam health sensor to monitor his nighttime erections and collect comprehensive data to formulate an Erection Health Score, also tracking aspects like sperm health and blood flow.
Bustamante argues that erections serve as a stronger benchmark than semen quality, as the latter varies greatly between individuals.

According to Dr. Welter, the foundation for promoting longevity and maintaining penis health largely revolves around diet, exercise, and overall metabolic wellness.
Bustamante advises focusing on a balanced diet rich in fruits, vegetables, and fiber while limiting red and processed meats, which can elevate blood pressure and contribute to ED.
Exercise plays a crucial role; Bustamante emphasizes the importance of muscle building, calling muscles “organs of longevity.” Aiming for 30 minutes of aerobic activity daily is key, though he cautions that cycling, for instance, has been linked to a higher incidence of ED due to pressure on sensitive areas.
On the treatment front, Bustamante notes that low doses of Cialis can promote smoother muscle relaxation and support erection functionality.
Are Biohacker Tools Worth It?
Over time, many men may experience reduced sensitivity due to factors like aging or sexual activity, which can hinder performance. Some biohackers have turned to low-intensity focused shockwave therapy, which aims to stimulate blood flow and tissue growth through micro-damage.
While the terms shockwave and sound wave therapy are often used interchangeably, they differ in intensity. “Sound therapy may not penetrate deeply enough to have a real effect,” Bustamante pointed out.
In contrast, Botox can provide short-term improvements in erectile quality by blocking nerve signals, though its effects typically last only a few months.
Penile stem cell injections are unapproved by regulatory bodies and remain considered experimental, without solid evidence to support their efficacy. Bask in caution regarding the emerging trend of injecting salmon derivatives; such practices pose risks for infections and other complications.
Instead, Bustamante advocates for treatments like the “P-shot,” utilizing platelet-rich plasma from the patient’s own blood to enhance blood circulation and tissue repair, contributing positively to one’s sex life.
