The cause of death for actor Nicholas Brendon, known for his role in the television series Buffy the Vampire Slayer, has been confirmed by a medical examiner in Indiana. Brendon passed away in March at the age of 54.
A coroner’s report indicates that Brendon’s death was due to natural causes, primarily resulting from cardiovascular disease. He had a significant history of smoking and had suffered a heart attack recently, which led to severe blockages in his coronary arteries and ultimately, cariogenic shock, along with organ failure.
On May 5, Coroner Todd Zeiner reported that Brendon’s passing was attributed to natural causes, specifically atherosclerosis and hypertensive cardiovascular disease, with acute pneumonia and a prior heart attack as contributing factors. Essentially, the primary issue was a 90% blockage of the right coronary artery.

Zayner mentioned that Teresa Fortier, a close friend of Brendon, was present when he became unwell and called 911. She noted that he had a persistent cough and was trying to self-medicate with over-the-counter drugs, while also experiencing chest pain. Despite her urging to seek hospital treatment, he declined. She expressed concern due to his long history of smoking and a recent back surgery, alongside a previous heart attack.
After finding no additional evidence at his residence, the coroner conducted an autopsy, revealing that Brendon had a notably enlarged heart, severe stenosis in his right coronary artery, moderate stenosis in both the left anterior descending artery and the left circumflex artery, as well as acute pneumonia and inflammation in the small intestine. The intestinal inflammation likely stemmed from ischemic changes related to the cardiogenic shock.
The heart attack referenced in the report took place in 2022. At that time, his social media relayed that he had been hospitalized for cardiac issues, including tachycardia and arrhythmia.
Brendon, a Hollywood native, first gained recognition for portraying Xander Harris in Buffy the Vampire Slayer, a role he maintained throughout most of the show’s 144 episodes across its seven seasons. Despite facing numerous health challenges and issues with addiction and the law, he consistently found work and continued to act in films until his passing.
