The Israel Defense Forces on Friday announced the findings of an investigation into the missile attack that killed several World Central Kitchen aid workers in the Gaza Strip.
IDF investigators said the incident “should not have happened” and said the authorities who authorized the attack mistakenly believed they were targeting Hamas operatives.

“The attack on the support vehicle was a serious error resulting from a serious failure due to mistaken identification, poor decision-making, and an attack that violated standard operating procedures,” the IDF said.
