The employer-based health insurance system in the U.S., which primarily covers those under 65, is facing a serious crisis. The share of working-age adults receiving insurance through their jobs has plummeted from 67% in 1998 to roughly 20% today. Many people are finding that options are becoming unaffordable, especially those with high out-of-pocket costs. Rising expenses for hospitals, doctors, and medications are squeezing profits and wages. In a climate of desperation, businesses are cutting traditional health benefits at a staggering rate.

This shift is fundamentally altering American expectations about job-related health coverage. The idea that employment should bring health protection can be traced back to World War II wage controls, resistance to government-run healthcare, and significant reforms in tax legislation.
