The Independent Office for Police Conduct (IOPC) says two police officers have been issued misconduct notices after a 17-year-old girl was struck and killed by a car after fleeing from a police car on the M5 in Somerset. did.

Tamzin fled from a marked police car that was stopped on the northbound road and was struck and killed by a car on the southbound road.
The IOPC said investigators spoke to witnesses and collected CCTV and body-worn camera footage.
The department said the officers had been issued notices of misconduct for “possible breaches of their duties and responsibilities.” It added that such notices inform police officers that their conduct is under investigation, but does not necessarily lead to disciplinary proceedings and is dependent on the outcome of the investigation.
IOPC Regional Director David Ford said: “Our thoughts and sympathies go out to Tamzin's family and friends and all those affected by the tragic events of that night. We met with the family to express our condolences and outline how the investigation will proceed. We will provide regular updates as inquiries continue.
“Our investigation is at an early stage and we are working hard to establish the exact circumstances of what happened, from the time of Tamzin's arrest to the subsequent development of events on the M5.”
On Wednesday, Tamzin's mother Amy Hall paid tribute to “the kindest, most caring, loving and loyal girl ever”.
