A recent study by the American Principles Project (APP) shows that taxpayer-funded gender transition surgery remains a vulnerable issue for Democrats, who most voters oppose.
“We wanted to help educate Americans, but primarily lawmakers spoke to the app’s executive director in light of where Americans, or how Americans feel about funding for gender identity programs, particularly on financing health services, particularly on congressional spending and settlement bills and all of them. “We want to make sure they know how unpopular these programs are in taxes.”
A survey conducted in early April with 1,500 respondents found that 43% of those who voted for former Vice President Kamala Harris in the last election were opposed to using taxpayer money to fund gender transition procedures, including surgery and hormonal treatment.

“That was a little higher than we generally see for Democrat voters,” Schilling said. APP is a socially conservative nonprofit advocacy group.
During the final months of President Donald Trump’s presidential campaign, his administration released high-budget ads targeting transgender women in women’s sports who may have helped move the needle along with swing voters.
The ad focused on Harris’ track record of introducing sex exchange procedures for incarcerated people in California.
In the survey, nearly 66% of Americans believe the federal government should not fund gender transition procedures, including adolescent blockers, hormones and surgery. The survey did not distinguish between minors and adults.

“Like elected Democrat officials, the upstream is going the other way around and it won’t work for them,” Schilling said. “Democrats are in a very tough place. They need to choose a base that is 20-30% of their taxes, which needs to support tax-based gender transitions and surgical payments, or swing voters that swing voters don’t want at all.”
In 2021, former President Joe Biden signed an executive order directing federal agencies to expand prevention protections to include sexual orientation and gender identity, including health care. He also reinterpreted Title IX to ensure the same protections applied within the educational environment.
The administration supported state-level initiatives under the Affordable Care Act, including Colorado’s inclusion of transgender treatment as essential health benefits.
“I think the more Donald Trump is talking about it and pays attention to it, the more people will be on the Republican side on this issue,” Schilling said.

Another question in the survey was, “If the November 2026 U.S. Congress general election was held today, I knew that if Democrat candidates were to support allowing gender transition procedures such as adolescent blockers, hormones, and surgery, I would support vote for Republican or Democratic candidates for U.S. Congress.”
52% of respondents said they would vote for Republican candidates, while 39% said they would support Democrats.

Since taking office, Trump has concluded “Extreme Gender Ideology,” cutting all federal funds for gender transition surgeries for minors and cuts through public health care. He also prohibited biological men from competing in female sports and required transgender individuals to serve according to the biological sex of the military.
