Rising Cases of Shigella in England

Shigella infections in England have surged, with reports indicating 2,560 cases in 2025, mainly affecting gay and bisexual men. The UK Health Security Agency (UKHSA) has raised alarms regarding the growing resistance to antibiotics in significant strains of this infection.

According to a recent UKHSA update, the figures for 2025 show a notable increase compared to previous years, with 2,052 cases in 2023 and 2,318 in 2024. Most cases were prevalent among men who have sexual relationships with other men.

Shigella is a bacteria-induced gut infection usually transmitted through contact with feces, which can occur during sexual activities, through direct contact, or via unwashed hands. Symptoms commonly include diarrhea (sometimes bloody), stomach cramps, and fever, often similar to those of food poisoning.

Dr. Katy Sinka, who leads the STI section at UKHSA, expressed concern about the uptick in sexually transmitted Shigella. She emphasizes that good hygiene during and after sexual activity, alongside consistent condom use, can significantly lower the risk of transmission.

She also stressed the importance of not ignoring symptoms and suggested that gay and bisexual men consult the NHS website for guidance on self-care and dehydration prevention.

Current Statistics

There are increasing worries about treatment challenges, with professionals from the Gastrointestinal Data Warehouse highlighting the rise in antibiotic resistance. Particularly concerning is that 86% of Shigella sonnei samples and 94% of Shigella flexneri samples showed high resistance.

Guidance on Testing and Recovery

Dr. Sinka noted that those tested positive for Shigella might also be at risk for other sexually transmitted infections, including HIV. Therefore, it is advisable to undergo a sexual health screening, either at a clinic or through online testing options.

For those diagnosed with Shigella, resting, staying hydrated, and avoiding sexual activities for at least seven days after symptoms cease is essential. It’s also recommended to steer clear of swimming pools, hot tubs, sharing towels, or preparing food for others until recovery is complete.

Sexual health services, including testing and treatment for STIs, are accessible through both clinics and online platforms. There has also been positive movement in the area of sexual health, as evidenced by the decrease in new HIV diagnoses among gay and bisexual men in England.
