Seeing your children marry is likely one of the most joyful moments for any parent. However, for one 65-year-old mother, her overwhelming happiness at her daughter’s wedding led to unexpected heart issues that required a trip to the emergency room.
Doctors reported this unusual case earlier this month, explaining that shortly after the wedding, the woman developed a rare condition called “happy heart syndrome.” Fortunately, she made a complete recovery.
“Our case highlights this rare occurrence and adds to the limited research documenting instances where intense joy corresponds with the sudden onset of stress cardiomyopathy,” the medical team noted in their publication in the Oxford Medical Case Reports.
Excessive joy
Happy heart syndrome, or HHS, is a unique variant of takotsubo cardiomyopathy, which itself is a rare heart condition.
This condition leads to a sudden weakening and ballooning of the heart’s left ventricle, which is the main chamber responsible for pumping blood throughout the body. The name takotsubo comes from the resemblance of the ventricle’s shape to a Japanese octopus trap. Typically, this condition is triggered by a significant emotional or physical stressor, though actual traumatic events—like a fall—can also be a factor. It’s often linked to deeply sad experiences, like a breakup or the death of a loved one, hence its nickname “broken heart syndrome.” However, in this instance, extreme joy was the catalyst.
In the report, it was noted that the woman sought medical attention three days after experiencing chest pain and shortness of breath, particularly with exertion, which is characteristic of typical angina. She had a prior history of hypertension and had visited a cardiology clinic a few months earlier for different chest pain that didn’t fit standard criteria, and her ECG had seemed normal. Despite this, doctors rushed her into intensive care, suspecting acute coronary syndrome, which can be life-threatening.
Upon examination, her coronary arteries appeared normal. However, her left ventricle exhibited the ballooning expected in takotsubo cardiomyopathy. Additional tests confirmed heart damage and reduced pumping efficiency. The woman attributed her symptoms to the “extreme joy” she experienced at her daughter’s wedding just days prior. Given that other causes were ruled out, doctors diagnosed her with happy heart syndrome.
A truly happy ending
HHS was only identified in 2016, more than 25 years after takotsubo cardiomyopathy was first recognized. Despite being rarer, the authors stress that medical professionals should remain vigilant about this condition. “It’s essential to maintain a high level of suspicion, even in patients without traditional cardiovascular risk factors,” they emphasized.
Takotsubo cardiomyopathy can be serious, and even fatal if not properly managed. A recent analysis of nearly 200,000 U.S. adults revealed that 6.5% of individuals with this condition died from it between 2016 and 2020. Thankfully, this case concluded positively.
The woman was discharged from the hospital and was able to return to her normal activities without any restrictions. Follow-up evaluations indicated her heart function had normalized.
“Despite the initially concerning situation, she made a full cardiac recovery and continues to lead a healthy life, eagerly anticipating the joyful milestones of her other children in the future,” the medical team concluded.
