Promising Vaccine for Pancreatic Cancer Under Research
A recent study at Johns Hopkins School of Medicine in Baltimore, Maryland, is showcasing a new vaccine that might help in preventing pancreatic cancer. The initial research, published in the journal Cancer Discovery, details a Phase 1 trial that involved 20 participants who were at genetic risk for pancreatic ductal adenocarcinoma (PDAC), which is known as the most prevalent and aggressive type of pancreatic cancer.
The individuals, though undiagnosed, had imaging tests revealing abnormalities typically seen in pancreatic cysts. During the trial, participants received a total of four injections: one at weeks 1, 3, and 5, followed by a booster at week 13. This vaccine, named mKRAS-VAX, specifically targets six common mutations in the KRAS gene, which accounts for over 90% of PDAC cases.
Results showed that 18 out of 20 participants (about 90%) produced an immune response aimed at the mutant KRAS. While responses varied, there was an average increase in immune activity of around 18-fold, and half of the participants responded to all six mutations included in the vaccine. Notably, the vaccine generated two types of T cells: one that attacks abnormal cells and another that supports long-term immune memory.
Participants appeared to tolerate the vaccine well, with only mild injection site reactions and temporary flu-like symptoms reported. Although the main focus of this trial was safety, it’s worth noting that no participants developed pancreatic cancer within approximately 16.5 months of follow-up. Furthermore, about 37.5% experienced either a decrease or complete disappearance of pancreatic cysts, which are significant risk factors for the disease.
Dr. Neha Zaidi, co-author of the study and associate professor of oncology at the Johns Hopkins Sidney Kimmel Comprehensive Cancer Center, remarked on the importance of the vaccine’s long-lasting immune responses. She emphasized that such responses are critical for preventing cancers that require sustained immunity. “This vaccine was safe and well-tolerated, supporting its potential use in larger cancer prevention studies,” she stated, noting this research provides the first proof of concept for a vaccine designed to combat pancreatic cancer in humans.
Another co-author, Dr. Michael G. Goggins, highlighted findings of pancreatic cyst stability or regression correlated with the vaccine-induced T cell responses. However, he suggested that more extensive studies are necessary to firmly establish the vaccine’s effectiveness.
Despite the promising early results, the study did have limitations, including its small sample size and the fact that it was more focused on safety than on clinical efficacy. Dr. Elizabeth Jaffee, also a senior author, pointed out the need for additional funding to further develop cancer prevention strategies, particularly for those at high risk. “This is vital as some cancers are becoming more prevalent and existing early detection methods are lacking,” she noted.
In conclusion, while there is still much to investigate, this study marks a significant step toward understanding how vaccines may help in the fight against pancreatic cancer.
